Terms of Service
The agreement between you and SickSlip Inc. (operating as SportSlip) when you use the service.

SportSlip is a service of SickSlip, Inc., a Delaware corporation (the "Company"). Clinical review and issuance of clearance documentation are provided exclusively by SickSlip P.A., a Florida professional association owned by Dr. Adam Z. Kawalek and contracted to the Company for the performance of professional medical services.
1. What SportSlip is — and isn't
SportSlip is an asynchronous telehealth service that provides a physician-reviewed pre-participation evaluation (PPE) clearance for children 5–18 who are participating in sports, camp, or school activities. A board-certified physician reviews the screening questionnaire, vitals, video observation, and uploaded documents, and either issues a signed clearance letter or refers your child for in-person care.
SportSlip is not:
- An emergency medical service. If your child has a medical emergency, call 911.
- A primary care relationship. We do not provide ongoing care, prescribe medications, treat existing conditions, or manage chronic disease.
- A substitute for an in-person physical exam. For some children — including those with complex cardiac history, recent concussion, or active medical issues — we'll refer to your pediatrician or a specialist instead of clearing.
- A diagnostic service. The screening + video review is a clearance evaluation, not a diagnosis.
2. Eligibility
- You must be at least 18 years old and the parent or legal guardian of the patient.
- The patient must be 5–18 years old.
- Both you and the patient must be physically present in a U.S. state where Dr. Kawalek holds an active medical license. Currently licensed in 29 U.S. states — see About Dr. Kawalek for the full list. Hawaii and New Jersey are the active gaps.
- You must provide accurate information. Knowingly withholding health information that's relevant to safe participation (cardiac symptoms, prior concussions, family sudden-death history, etc.) voids the clearance.
3. Pricing
- Standard clearance: $59 — 24-hour turnaround.
- Rush clearance: $79 — same-day during business hours.
- ECG cardiac rhythm screen add-on: +$19 — requires a compatible device (Apple Watch Series 4+, Samsung Galaxy Watch 3+, AliveCor Kardia, Fitbit Sense/Charge 6, Withings ScanWatch).
All prices are in U.S. dollars and exclude any applicable taxes. Payment is processed by Stripe; we don't see or store your card number.
4. Refund policy
- Full refund if we can't clear your child. If after reviewing the screening + video + uploads Dr. Kawalek determines that telehealth clearance isn't appropriate (e.g., a cardiac concern that warrants in-person evaluation), we refund the full purchase price and provide a free referral letter to take to your pediatrician or a cardiologist.
- 50% refund if your camp, school, or sports clinic won't accept the clearance. Just tell us within 30 days of issuance which institution declined it — no proof required. Because the clinical evaluation was performed and the signed clearance is yours to keep, half the fee is retained.
- Full refund if you withdraw before review. You can cancel any time before Dr. Kawalek has reviewed your case for a full refund. After review begins, we evaluate refunds on a case-by-case basis — the visit IS the service.
- ECG add-on: refundable if Dr. Kawalek can't read the recording due to recording quality issues. Not refundable if the recording is usable but you change your mind.
5. Your responsibilities
- Provide accurate, complete information about your child's health history.
- Confirm that the screening matches your child's current state (not a snapshot from 6 months ago).
- Record the video honestly — your child performing the requested maneuvers, not a substitute.
- Acknowledge that a clearance is valid for 12 months from issuance, and that material health changes during that period (new diagnoses, significant injuries, etc.) invalidate the clearance and require a fresh evaluation.
6. Intellectual property
SportSlip's software, documentation, branding, and educational content are owned by SickSlip Inc. You receive a personal, non-transferable license to use the service for your child's clearance. The clearance letter, completed form, and superbill we generate are yours to use, forward, print, and submit to insurance for reimbursement.
7. Disclaimers + limitation of liability
The service is provided "as is." To the maximum extent permitted by law, SickSlip Inc.'s aggregate liability for any claim arising out of the service is limited to the amount you paid SportSlip in the prior 12 months. This limitation does not apply to claims arising from gross negligence, willful misconduct, or where prohibited by law.
Nothing in these Terms limits your rights under applicable consumer protection law (e.g., California CCPA, Massachusetts Consumer Protection Act).
8. Termination
You may terminate your account at any time by emailing adam@sportslip.co. We may terminate or suspend access if you violate these Terms, provide false information, or use the service to defraud (e.g., requesting a clearance for someone you're not the legal guardian of). Termination does not refund prior charges except as set out in §4.
9. Governing law + disputes
These Terms are governed by the laws of the State of Delaware(the state of incorporation of SickSlip, Inc.). Clinical services are provided by SickSlip P.A. under the medical-practice laws of the state where Dr. Kawalek's relevant license is held and where the patient is physically located at the time of service. Disputes arising from the platform itself (account, payment, technology) are resolved under Delaware law; disputes arising from the provision of clinical care are resolved per the law of the patient's state of residence. Nothing in this section limits state-specific consumer protection rights that override by law (e.g., California CCPA, Massachusetts Consumer Protection Act).
10. Changes
We may revise these Terms. Material changes will be emailed to active customers and posted here with an updated "Last reviewed" date. Continued use of SportSlip after notice constitutes acceptance.
